Towards optimizing the host response to biotics: a report of an International Scientific Association for Probiotics and Prebiotics working group

Authors produced this report from an ISAPP discussion group at the 2025 annual meeting, focusing on exploring key targets that mediate a host’s response to biotics. In this current era of increased knowledge about mechanisms involving the human gut and its microbiome, the group discussed whether biotics can be identified as candidates for a specific indication using a more informed, systematic approach. They describe 5 host systems that can be the target of modulation by biotics, and then align on 5 recommendations for candidate biotic interventions going forward: the interventions should (1) modulate host metabolism or the immune system in a specific way; (2) consider using metabolites as readouts or therapeutic targets; (3) determine how response depends on spatial location within the gut, host genetics or diet, and environmental influences; (4) explore various microbial fermentation substrates; and (5) leverage new technologies and methods to complement human studies. The authors hope these recommendations will enhance the rapidity with which biotic research is translated into clinical applications.
